PatientKeeper Redefines Medication Reconciliation with First Physician-Friendly Med Rec Software Application Within a Complete Physician Workflow Environment
PatientKeeper streamlines medication reconciliation with an application that engages physicians, maintains nursing workflow, and overlays existing hospital information systems
WALTHAM, MA – Feb. 21, 2012 – PatientKeeper®, Inc., the leading provider of physician healthcare information systems, today announced availability of the latest release of the industry’s first physician-friendly medication reconciliation software application offered within a complete physician workflow environment. PatientKeeper Medication Reconciliation™ makes it easy for physicians and other clinicians to electronically track and transition patient medications from home to hospital at time of admission, and back to the home at time of discharge.
PatientKeeper Medication Reconciliation can be used as a stand-alone application or in combination with PatientKeeper CPOE™. PatientKeeper Medication Reconciliation reflects how physicians naturally work through medication reconciliation; physicians can clearly see the starting point – for example, a patient’s regimen of medications taken at home – and the changes at the next level of care in a single view. PatientKeeper’s software lets physicians continue, discontinue or hold a patient’s existing medication in a single step, and often with a single tap. For instance, modifying an existing order requires only that the physician specify the change; PatientKeeper then automatically discontinues the existing order and creates a new order reflecting the change.
As with all PatientKeeper software, PatientKeeper Medication Reconciliation integrates with hospitals’ existing back-end systems. As a result, physicians can go to one place for all their patient information and activities; and hospitals don’t have to rip and replace their existing technology infrastructure to create an IT environment that appeals to physicians and encourages adoption and sustained Meaningful Use.
“PatientKeeper Medication Reconciliation is wonderful,” said Sara Houser, MSN, Chief Nursing Officer at Ashe Memorial Hospital and the project team leader for the hospital’s PatientKeeper CPOE and PatientKeeper Medication Reconciliation software implementation. “Physicians like it, and it fits seamlessly with our nursing workflow. PatientKeeper integrates with our nursing system, so we continue doing what we’ve been doing; only now the physicians can handle their portion electronically, so the entire reconciliation process is faster. And at discharge, PatientKeeper lets us automatically produce a full order reconciliation for patients, including meds, labs, nursing and dietary orders.”
