Integration & Access

Community Connector

Establish Efficient Health Information Exchange (HIE)

The exchange of patient health information across diverse settings and multiple providers is critical to improving the quality, safety, cost and efficiency of healthcare. The PatientKeeper Community Connector provides physicians and healthcare providers with real-time access to a patient’s longitudinal health record and enables the exchange of information between hospitals, labs, physician practices and other providers across healthcare communities.

Meet ARRA HIE Requirements to Qualify for HITECH Stimulus Incentives

ARRA legislation requires not only the establishment of electronic medical records, but the ability to maintain those records and make them accessible to all stakeholders, including patients, on demand. PatientKeeper Community Connector helps hospitals and physician practices accomplish this goal quickly and easily, without requiring additional investments in technology infrastructure.

Improve Patient Care with Access to Information and Better Communication

The Connector enables all members of a healthcare team to stay up to date on a patient’s condition in real time because they are all working from the same information. The system seamlessly manages the secure exchange of clinical data including lab and test results, allergies, medications, clinical notes and summaries. Patient medical records are automatically updated and immediately accessible using the local system’s patient ID, so physicians receive information that is immediately accessible and useful to them.

Maintain Data Integrity, Security and Connectivity

Our technology offers a secure, fully customizable framework to extract, send and exchange comprehensive clinical information utilizing your existing infrastructure, which saves you time and money, and achieves your health information exchange (HIE) goals.

  • Flexible interface options control the frequency and method of transport for updates – data transfer can occur in timed batches, on demand, or based on an event/trigger
  • Secure Access File Exchange (SAFE) Broker eliminates the need for complex, expensive point-to-point VPNs
  • Secure channel communication uses SSH (Secure Shell) security protocol and double-encryption methods (encrypting commands and data), insuring the highest level of security and confidentiality
  • Data Normalization ensures proper visual integration from all contributing sources, including: LOINC, ICD, NDC, SNOMED and others
  • Community EMPI exchanges data using each office’s local patient ID
  • Support for multiple file formats, including HL7, CSV, XML and CCR/CCD increases interoperability and connectivity to more data sources
  • Data transfer can be customized by multiple criteria, including: provider role, patient type, test type, status or patient participation

 Incorporate Workflow Support

Connectivity without workflow support severely limits the utility of health information exchange. The PatientKeeper Physician Portal provides a thin client view of aggregated data. Physicians, their staff and other clinicians can review, trend and filter information customized for their specialty and workflow, in addition to taking action on that information through a series of workflow applications. In the community or in the hospital, PatientKeeper supports physicians by providing both the exchange of data into their EMR as well as a comprehensive view of the chronological and longitudinal records across the continuum of care.

A platform you can build on

Community Connector is part of the PatientKeeper suite of applications that improve physician workflow and connectivity to the extended community of healthcare providers without replacing existing IT systems. PatientKeeper's extensible platform includes applications that automate charge capture; reviewing and signing charts; collaboration with the entire healthcare team; viewing clinical results; writing e-prescriptions; and managing patient discharge.
