PatientKeeper Medication Reconciliation enables physicians to view the medication history for their patients – providing medication lists and allergies in report format at the point of care. With PatientKeeper Medication Reconciliation, physicians and hospitals are able to improve patient safety by avoiding mistakes, duplications and omissions as medication information can be viewed when entering/placing orders. The PatientKeeper Medication Reconciliation application has been developed to help fulfill the Joint Commission’s National Patient Safety Goal to accurately and completely reconcile medications across the continuum of care.

Most institutions currently have a paper-based method for capturing and documenting the reconciliation process. The admission reconciliation involves patient/family interview by the nurse or physician to collect an up-to-date medication history. This practice can lead to the potential for medication errors – which is currently the third leading cause of death in the United States.

Streamline workflow

PatientKeeper Medication Reconciliation integrates with DrFirst via the PatientKeeper Physician Portal. With PatientKeeper’s Medication Reconciliation application, users are able to view integrated medication lists and allergies for patients across communities by connecting to data from RxHub, DrFirst, and SureScripts. Physicians are able to search patient history at the point of care and view a list of possible medications that a patient may be taking. The physician is then able to analyze the prior medication history (up to 10 years) and understand possible drug interactions, discover duplicate therapies, and measure compliance.
